Social and Recreational Activities (K.S.A. 44-508(f)(3)(C)) “The words, “arising out of and in the course of employment’’ as used in the workers compensation act shall not be construed to include injuries to employees while engaged in recreational or social events under circumstances where the employee was under no duty to attend and where the injury did not result from the performance of tasks related to the employee’s normal job duties or as specifically instructed to be performed by the employer.”

When AN EMPLOYEE IS INJURED AT WORK:
ADMINISTRATOR:
- Notify Matt Hirsch, Central Office, when a staff member is injured.
- Complete Supervisor's Accident Investigation Report.
- Send both Supervisor and Employee incident reports to Matt Hirsch, Central Office.
EMPLOYEE:
- Complete Report by Injured Employee.
- Complete Report by Eye Witness (if applicable).
WHEN AN EMPLOYEE NEEDS MEDICAL TREATMENT:
ADMINISTRATOR:
- Complete Temporary Prescription ID Card. Employee takes this to a listed Pharmacy if the doctor prescribes a medication when they receive initial treatment.

- If you need to be seen by a doctor:
- 8am-4pm M-F: Contact Lisa Devena at 730-5010 to schedule an appointment at Stormont-Vail Work Care, 1516 SW 6th Ave. Topeka, KS 66605
- 4pm-8pm M-F: Report to the Stormont-Vail Express Care, at 2929 S.E. Walnut Drive.
- Weekends 11am -5pm: Report to Stormont-Vail Express Care, at 2909 S.E. Walnut Drive.
- If by ambulance or after hours please report to the Stormont-Vail Emergency Room.
- If you need a prescription filled, take the Temporary Prescription ID Card to one of the pharmacies listed on the back page of the form.
WHEN AN EMPLOYEE RETURNS TO WORK:
EMPLOYEE:
- Present medical release from a medical care provider to your supervising administrator before returning to work.
- A physician should note any work restrictions on the release.
ADMINISTRATOR:
- Forward medical release to Matt Hirsch, Central Office.
